AI output looks polished. Specific numbers, clean formatting, confident tone. But when it doesn't know your project, it fills the gaps with made-up context. A PM on my team wrote a PRD claiming we'd "cut support tickets 50%." We had zero support tickets. The feature hadn't launched. That's the cost: your work looks sloppy and you don't even know it.
